Penile Prosthesis A medical device surgically implanted in the penis to achieve an erection
A penile prosthesis is a medical device surgically implanted in the penis to help men with erectile dysfunction (ED) achieve and maintain an erection sufficient for sexual activity.
This treatment is indicated in cases of severe erectile dysfunction that do not respond to other treatments such as oral medications, penile injections, and vacuum devices or in specific medical conditions such as Peyronie's disease with associated erectile dysfunction, or after certain pelvic surgeries.
There are two types of implantable penile prostheses:
- Malleable (Semi-rigid) Prosthesis
Consists of flexible rods implanted in the penis, keeping it in a semi-rigid state, which can be manually adjusted upward or downward.
It offers ease of use and a lower likelihood of mechanical failure.
- Inflatable Prosthesis
Comprises two or three components: inflatable cylinders (implanted in the penis), a fluid reservoir (placed in the abdominal area) and a pump (placed in the scrotum).
The patient uses the pump to move fluid from the reservoir to the cylinders, creating an erection.
It provides a more natural appearance and feel of the penis, both in flaccid and erect states.
The surgical procedure is performed under general anesthesia and typically takes 1 to 2 hours, usually involving an incision at the base of the penis or the lower abdomen.
The recovery period generally ranges from 4 to 6 weeks before resuming normal activities. During this period, sexual activity and intense physical effort should be avoided. Antibiotics are also used to prevent the risk of postoperative infections.
The main benefits of this treatment include:
- It is a permanent solution for treatment of erectile dysfunction;
- It has a high satisfaction rate for both patients and their partners;
- It offers significant improvement in sexual relations and quality of life.
The main risks associated with this surgical procedure are:
- Development of postoperative infection;
- Formation of internal scar tissue;
- Mechanical failure of the prosthesis (more common with inflatable prostheses);
- Need for revisions or replacements of the prosthesis over time.
The penile prosthesis is an effective and durable option for men with severe erectile dysfunction. Although it involves surgery, with associated risks, it often results in significant improvement in sexual function and quality of life.
The decision to undergo the implant procedure and the choice of prosthesis type should be made in consultation with an experienced urologist, considering the patient's individual needs and conditions.
